Social Worker - Care Leavers team - Newcastle

We are seeking a passionate and dedicated Social Worker to join our Care Leavers team in Newcastle. This role is pivotal in providing support and guidance to young individuals transitioning from care to independent living. As a Social Worker, you will work collaboratively with young people, their families, and various stakeholders to ensure a smooth transition and promote their well-being and development.

Your key responsibilities will include:

  • Conducting assessments to identify the needs and strengths of care leavers.
