As a QSW in this position, you will play a crucial role in supporting and safeguarding vulnerable individuals and families within the community.

Key responsibilities and duties:

Conducting assessments and developing care plans for individuals in need of support Providing direct intervention and support to service users, promoting independence and well-being Collaborating with multi-disciplinary teams and external agencies to ensure holistic care for clients Participating in case conferences, reviews, and court proceedings as required Maintaining accurate and up-to-date records of interventions and progress

Essential qualifications and skills:

Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC) Demonstrated experience in working within a similar role, preferably within an integrated neighbourhood setting Strong assessment and decision-making skills, with the ability to work autonomously and as part of a team Excellent communication and interpersonal skills, with a compassionate and empathetic approach Sound knowledge of relevant legislation and policies governing social work practice
