Mental Health Hospital Discharge Team Social Worker | East Sussex | £38 per hour

Caritas Recruitment is currently seeking an experienced and dedicated Social Worker to join the Mental Health Hospital Discharge Team in East Sussex. This role offers an exciting opportunity to contribute to the timely and safe discharge of individuals from hospital settings, working with a dynamic team committed to providing excellent care and support.
As a Social Worker in this Countywide team, you will focus on working with both working-age adults (18-70) and older adults (70+). You will play a key role in supporting hospital discharges across all acute hospitals in East Sussex for individuals admitted due to mental health needs. This new initiative involves close collaboration with Sussex Partnership Foundation Trust (SPFT), the voluntary sector, and Adult Social Care services to ensure seamless, creative discharge planning with the adult at the centre of care.
Key ResponsibilitiesWork as part of a remote partnership team, collaborating with SPFT and the voluntary sector to support safe and timely hospital discharges.
Develop discharge plans in partnership with service users, families, and professionals, ensuring that the adult's needs and preferences are at the heart of decision-making.
Undertake assessments to ensure individuals are safely supported post-discharge and that services are in place to meet their ongoing care needs.
Build and maintain positive relationships with clients, family members, and multi-agency professionals.
Provide advice, guidance, and support to individuals and families throughout the discharge process.
What We're Looking ForA Social Worker registered with Social Work England (please include your registration number in your application).
Completed the Assessed and Supported Year in Employment (ASYE).
Strong communication and relationship-building skills to work effectively with clients, families, and professionals.
Motivated and proactive approach to work, with a commitment to personal and professional development.
Ability to meet the travel requirements of the post across East Sussex.
Experience or a keen interest in mental health services and hospital discharge processes.
