We are seeking a dedicated and knowledgeable Specialist Assessment Worker to join our Newcastle team. This non-caseholding role is essential in providing high-quality assessments for individuals in need of social care services. You will work collaboratively with a variety of stakeholders, including families, healthcare professionals, and community organizations, to ensure that individuals receive the necessary support tailored to their unique circumstances.
Key responsibilities in this role include:
Conducting comprehensive assessments of individuals' needs and circumstances. Developing and implementing appropriate action plans that address identified needs. Providing expert advice and guidance to social work teams and other professionals. Maintaining accurate and timely documentation of assessments and outcomes. Carrying out initial assessments and developing comprehensive support plans. Engaging with families and children to gather information and understand their needs. Working closely with multi-agency partners to coordinate services and interventions. Maintaining accurate and up-to-date case records and documentation.
