We are seeking a dedicated and compassionate Qualified Social Worker to join our Older Peoples Team in Wokingham. This role is pivotal in providing support and guidance to older adults, ensuring they receive the care and assistance they need to maintain their independence and well-being. Key responsibilities include:
Conducting comprehensive assessments to identify the needs and preferences of older adults. Developing and implementing person-centered care plans in collaboration with service users and their families. Providing ongoing support and advocacy for clients, ensuring their voices are heard in decision-making processes. Monitoring and reviewing care plans regularly to ensure they remain effective and relevant. Working closely with health professionals and community organizations to coordinate services and resources.To be considered for this role, candidates must possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work or equivalent). Registered with Social Work England. Proven experience in adult social work, particularly with older populations. Strong assessment and analytical skills, with the ability to develop effective care plans.
