We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Mental Health Team in Salford. This role is pivotal in providing high-quality support to adults experiencing mental health challenges. As a key member of our multidisciplinary team, you will work collaboratively to assess needs, develop care plans, and ensure that individuals receive the appropriate services to enhance their quality of life.
Your key responsibilities will include:
Conducting comprehensive assessments of adults with mental health needs to determine their eligibility for services. Developing, implementing, and reviewing person-centered care plans in collaboration with service users and their families. Providing ongoing support and guidance to individuals, ensuring they have access to necessary resources and services. Collaborating with other professionals, agencies, and community resources to facilitate effective interventions. Maintaining accurate and timely documentation of assessments, care plans, and progress notes in accordance with organizational policies.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly within mental health settings. Strong assessment, analytical, and intervention skills.
