As a member of the Transformation Team, your key responsibilities will include:

Conducting comprehensive assessments to identify the needs and strengths of individuals. Developing and implementing tailored support plans in collaboration with service users and their families. Facilitating access to services and resources, ensuring individuals receive the support they need. Advocating for the rights and interests of service users within the community. The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England.
