Social Worker - Hospital Discharge Service - Hammersmith & Fulham

We are seeking a dedicated and compassionate Social Worker to join our Hospital Discharge Service in Hammersmith & Fulham. This vital role is designed to facilitate smooth transitions for patients from hospital to home, ensuring they receive the necessary support and resources for a successful recovery. You will work collaboratively with healthcare professionals, patients, and their families to assess needs, coordinate care, and provide ongoing support.

Key responsibilities include:

  • Conducting assessments of patients’ social, emotional, and practical needs prior to discharge.
  • Developing and implementing discharge plans in collaboration with multi-disciplinary teams.
  • Providing information and advice to patients and families regarding available services and resources.
  • Advocating for patients’ rights and ensuring they receive appropriate care post-discharge.
  • Monitoring and evaluating the effectiveness of discharge plans and making necessary adjustments.

The ideal candidate will possess the following qualifications and skills:

  • A recognized Social Work qualification (Degree in Social Work or equivalent).
  • Current registration with Social Work England.
  • Experience in hospital discharge planning or a related field is highly desirable.
  • Strong communication and interpersonal skills with the ability to work effectively in a team.
  • Excellent problem-solving abilities and a proactive approach to care coordination.
