Caritas Recruitment is recruiting an experienced Adult Social Worker to join the Hospital Discharge Service based at Charing Cross Hospital.

This is an excellent opportunity to work within a fast-paced multidisciplinary hospital setting, supporting adults, their families and carers to achieve safe and effective discharges from hospital. You will undertake complex assessments, safeguarding enquiries, mental capacity assessments and support planning, promoting independence and positive outcomes in line with the Care Act 2014.

Key Responsibilities

  • Manage a complex caseload within a hospital discharge environment.
  • Complete Care Act assessments, support planning and reviews.
  • Undertake Mental Capacity Assessments and Best Interest decisions.
  • Work closely with NHS professionals, families and partner agencies to facilitate safe discharges.
  • Promote strengths-based practice, independence and wellbeing.
  • Support safeguarding enquiries and risk management.
  • Provide professional guidance and support to less experienced colleagues where required.

Requirements

  • Social Work England registration.
  • Minimum 3 years' post-qualified Adult Social Work experience.
  • Strong knowledge of the Care Act 2014, Mental Capacity Act and safeguarding procedures.
  • Experience within hospital discharge, community, learning disability or mental health services.
  • Excellent assessment, communication and multi-agency working skills.
