Key Responsibilities

  • Manage a caseload focused on hospital discharges and reassessments
  • Undertake Care Act assessments, reviews, and capacity assessments
  • Liaise with multidisciplinary teams, including NHS colleagues and community services
  • Support individuals and families to achieve safe and sustainable discharge outcomes
  • Apply a strengths-based, person-centred approach in line with best practice

What We're Looking For

  • Qualified Social Worker (Social Work England registered)
  • Experience within hospital discharge teams or similar adult services settings
  • Strong knowledge of the Care Act, MCA, and safeguarding frameworks
  • Ability to work flexibly across various hospital sites in East Yorkshire
  • Excellent communication and partnership-working skills
