Hospital Social Worker -East Yorkshire-Discharge/Reassessment
Location: East Yorkshire (across multiple hospital sites)
Contract: Long-term contracts available
Key Responsibilities
- Manage a caseload focused on hospital discharges and reassessments
- Undertake Care Act assessments, reviews, and capacity assessments
- Liaise with multidisciplinary teams, including NHS colleagues and community services
- Support individuals and families to achieve safe and sustainable discharge outcomes
- Apply a strengths-based, person-centred approach in line with best practice
What We're Looking For
- Qualified Social Worker (Social Work England registered)
- Experience within hospital discharge teams or similar adult services settings
- Strong knowledge of the Care Act, MCA, and safeguarding frameworks
- Ability to work flexibly across various hospital sites in East Yorkshire
- Excellent communication and partnership-working skills
