We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Reablement Service team in Islington. As a key member of the team, you will be responsible for providing high-quality support to individuals in the community who are working towards regaining their independence and improving their overall well-being.

Key responsibilities and duties include:

  • Conducting assessments and developing personalized care plans for service users
  • Delivering practical and emotional support to promote reablement and independence
  • Working collaboratively with other professionals and agencies to ensure holistic care
  • Monitoring progress and making adjustments to care plans as needed
  • Advocating for service users and empowering them to make informed decisions about their care

The ideal candidate for this role will possess the following essential qualifications and skills:

  • Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC)
  • Demonstrated experience in working with vulnerable adults and promoting independence
  • Excellent communication and interpersonal skills
  • Strong assessment and care planning abilities
  • Ability to work effectively within a multidisciplinary team

In addition to the essential qualifications and skills, candidates with experience in reablement services or a similar field will be highly regarded.
