We are seeking a dedicated and passionate Children’s Assessment Practitioner to join our dynamic Assessment Team in Bolton. This role is crucial in ensuring the safety and well-being of children and families within our community. As a Children’s AP, you will conduct thorough assessments, develop care plans, and work collaboratively with families and other professionals to provide the best outcomes for children in need.
Your key responsibilities will include:
Conducting comprehensive assessments of children and families to identify needs and risks. Developing and implementing effective intervention strategies and care plans. Maintaining accurate and up-to-date case records and documentation. Collaborating with multi-agency teams to ensure a holistic approach to child welfare. Providing support and guidance to families, empowering them to make positive changes.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in children’s social work, particularly in assessment and intervention. Strong communication, interpersonal, and report writing skills.
