We are currently seeking a dedicated and experienced Qualified Social Worker to join our Children's Disability Service team in Blackburn. As a key member of the team, you will be responsible for providing high-quality support and intervention to children and families with disabilities, ensuring their well-being and promoting their independence.

Key responsibilities and duties:

  • Assessing the needs of children with disabilities and developing care plans in collaboration with the child, family, and other professionals
  • Providing direct support and intervention to children and families, including advocacy and guidance on accessing appropriate services
  • Monitoring and reviewing the progress of children and families, adjusting care plans as needed to ensure positive outcomes
  • Participating in multi-disciplinary team meetings and case conferences to coordinate care and support services

Essential qualifications and skills:

  • Qualified Social Worker with a valid HCPC registration
  • Experience working in a similar role within children's services, preferably with a focus on disabilities
  • Strong assessment and intervention skills, with the ability to work collaboratively with children and families
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust with diverse populations
