We are currently seeking a dedicated and experienced Adult's Social Worker to join the Reablement Team in the vibrant London Borough of Islington. As a valued member of our team, you will play a crucial role in supporting adults to regain their independence and enhance their quality of life.

Key responsibilities and duties:

Conducting assessments of individuals' needs and developing personalized care plans Providing practical support and guidance to service users in their homes or community settings Collaborating with healthcare professionals, families, and external agencies to ensure holistic care Monitoring progress and making necessary adjustments to reablement plans Advocating for service users' rights and promoting their well-being

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England Proven experience in working with adults in a social care setting Excellent assessment and communication skills Ability to work autonomously and as part of a multidisciplinary team Strong commitment to promoting independence and empowerment

Additional requirements or benefits:
