We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Reablement Service team in Islington. As a key member of the team, you will be responsible for providing high-quality support to individuals in the community who are working towards regaining their independence and improving their overall well-being.

Key responsibilities and duties include:

Conducting assessments and developing personalized care plans for service users Delivering practical and emotional support to promote reablement and independence Working collaboratively with other professionals and agencies to ensure holistic care Monitoring progress and making adjustments to care plans as needed Advocating for service users and empowering them to make informed decisions about their care

The ideal candidate for this role will possess the following essential qualifications and skills:

Qualified Social Worker with current registration with the Health and Care Professions Council (HCPC) Demonstrated experience in working with vulnerable adults and promoting independence Excellent communication and interpersonal skills Strong assessment and care planning abilities Ability to work effectively within a multidisciplinary team

In addition to the essential qualifications and skills, candidates with experience in reablement services or a similar field will be highly regarded.
