We are currently seeking a dedicated and experienced Qualified Social Worker to join our Referral & Assessment Team in Wokingham. As a key member of the team, you will be responsible for conducting assessments, making referrals, and providing support to vulnerable individuals and families in need.

Key responsibilities and duties include:

  • Conducting thorough assessments to identify the needs and risks of service users
  • Developing and implementing care plans in collaboration with service users and their families
  • Making appropriate referrals to internal and external support services
  • Providing crisis intervention and support to individuals in urgent situations
  • Maintaining accurate and up-to-date case records and reports

The ideal candidate for this position will possess:

  • A recognized Social Work qualification and registration with Social Work England
  • Proven experience working in a similar role within a statutory social work setting
  • Strong assessment and analytical skills with the ability to make sound decisions under pressure
  • Excellent communication and interpersonal skills to engage effectively with service users and stakeholders
  • Ability to work autonomously and as part of a multidisciplinary team
