The London Borough of Merton is seeking a dedicated and passionate Adult's Social Worker to join our Transitions Team. This role is vital in supporting individuals with complex needs as they transition into adulthood. You will work collaboratively with service users, their families, and multi-disciplinary teams to ensure the delivery of high-quality care and support tailored to individual needs.

Key responsibilities and duties include:

  • Conducting comprehensive assessments of individuals transitioning from children’s services to adult services.
  • Developing, implementing, and reviewing care plans in partnership with service users and relevant stakeholders.
  • Providing support and guidance to families and carers throughout the transition process.
  • Ensuring compliance with relevant legislation, policies, and procedures in social work practice.
  • Maintaining accurate and timely documentation of casework and interventions.
  • Participating in team meetings, supervision, and professional development opportunities.

To be considered for this position, candidates must possess the following essential qualifications and skills:

  • A recognized social work qualification (BA/MA in Social Work or equivalent).
  • Current registration with Social Work England.
  • Experience working with adults with complex needs, particularly in a transitions context.
  • Strong assessment and analytical skills, with the ability to make informed decisions.
  • Excellent communication and interpersonal skills, demonstrating empathy and understanding.
  • Ability to work effectively within a multi-disciplinary team and manage a varied caseload.
