Transitions Social Worker - Staffordshire

Are you a qualified Adult Social Worker who thrives on meaningful work and real impact? Join a dynamic Transitions Team supporting young people with disabilities, neurodiversity and mental health needs as they move into adulthood.

This is a fast‑paced, rewarding role where your expertise will directly shape outcomes for young people and their families across Staffordshire.

What You'll Be Doing

  • Managing a varied Transitions caseload, including reviews, safeguarding enquiries (S42) and late referrals
  • Completing strengths‑based Adult Social Care assessments under the Care Act 2014
  • Working within the Mental Capacity Act, Mental Health Act, and DoLS
  • Supporting young people with learning disabilities, autism and co‑morbid mental health needs
  • Developing outcome‑focused support plans that promote independence and choice
  • Identifying and managing risk confidently and proportionately
  • Working closely with families, carers, providers and multi‑disciplinary partners
  • Contributing to MAPPA, MARAC and S117 work where required
  • Keeping clear, high‑quality case records and contributing to team performance targets

What We're Looking For

  • Qualified Social Worker with Social Work England registration
  • Experience in Adult Social Care (Transitions experience ideal but not essential)
  • Strong knowledge of legislation and confident safeguarding practice
  • A motivated, person‑centred professional who delivers quality work
