Job Overview:We are seeking a dedicated and compassionate Adults Social Worker to join our Learning Disabilities (LD) Team. The successful candidate will be responsible for providing high-quality social work support to adults with learning disabilities, promoting their independence, well-being, and social inclusion. This role requires strong assessment, advocacy, and partnership-working skills to ensure individuals receive the necessary care and support tailored to their needs.Key Responsibilities:Conduct strength-based assessments and develop person-centered care plans in line with the Care Act 2014.Work collaboratively with healthcare professionals, families, and partner agencies to ensure coordinated support.Support individuals with transition planning, ensuring continuity of care and support when moving between services.Conduct Mental Capacity Assessments and implement Best Interests Decision-making processes where applicable.Provide safeguarding interventions to protect vulnerable adults from abuse or neglect.Ensure compliance with legislative frameworks, including the Mental Health Act 1983 (amended 2007), Mental Capacity Act 2005, and the Care Act 2014.Maintain accurate and timely case records and reports using relevant IT systems.Support service users in accessing community resources, housing, employment, and educational opportunities.Contribute to service development, innovation, and quality improvement initiatives.Essential Criteria:Qualified Social Worker with a Degree or Master's in Social Work.Registration with Social Work England.Experience working with adults with learning disabilities in a social care setting.Strong knowledge of legislation, policies, and best practices in adult social work.Experience in carrying out risk assessments, safeguarding investigations, and care planning.Ability to work effectively within a multi-disciplinary team.Excellent communication, advocacy, and interpersonal skills.A proactive and compassionate approach to empowering service users.Competence in IT systems and case management software.A valid UK driving license (desirable) Caritas Recruitment is acting as an Employment Business in relation to this vacancy.
