Adults Social Worker - Transitions team
Are you an experienced Qualified Social Worker with a passion for supporting young people moving into adulthood?
We're recruiting for a Transitions Social Worker in Stoke-on-Trent, working with young people (typically 14-25) as they move from Children's Services into Adult Social Care - a critical stage where your input directly shapes long-term outcomes.
🌟 The Role
You'll play a key role in supporting young people with additional needs - including learning disabilities and complex needs - ensuring smooth, well-planned transitions into adult services.
This is a highly rewarding position where early planning, strong relationships and multi-agency working make a real difference.
✅ Essential Requirements (Non-Negotiable)
- Qualified Social Worker (Social Work England registered)
- Proven experience working as a Social Worker
- Experience working with young people, transitions, or adult services
- Strong understanding of the Care Act, safeguarding and Mental Capacity Act
🚫 This is not suitable for newly qualified or stepping-up candidates - you must be confident managing cases independently.
💼 Key Responsibilities
- Support young people transitioning from Children's to Adult Services
- Complete strengths-based assessments and transition planning
- Work closely with families, education, health and partner agencies
- Manage safeguarding concerns and risk effectively
- Promote independence, wellbeing and positive long-term outcomes
- Ensure clear, defensible decision-making and recording
You'll be joining a service focused on early planning, person-centred practice and meaningful outcomes.
