  • Conducting assessments to identify the needs and aspirations of young adults transitioning from children’s services.
  • Developing and implementing tailored support plans in partnership with young adults, families, and other professionals.

  • Monitoring and reviewing the effectiveness of support plans, making adjustments as necessary.
  • Maintaining accurate records and documentation in line with statutory requirements and best practice standards.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (BA/MA in Social Work or equivalent).
  • Registration with Social Work England.
  • Experience working with young adults, particularly in a transition or support role.
  • Strong communication and interpersonal skills, with the ability to build rapport and trust.
  • Knowledge of relevant legislation, policies, and best practices in social work.
