The London Borough of Tower Hamlets is seeking a dedicated and experienced Adult's Social Worker to join our Assessment Team. This vital role involves working with vulnerable adults to assess their needs and support their wellbeing. Key responsibilities include:

  • Conducting comprehensive assessments of adults’ needs and risks, ensuring that all relevant information is gathered and recorded accurately.
  • Developing and implementing person-centred support plans in collaboration with service users and their families.
  • Working closely with multi-disciplinary teams and external agencies to coordinate services and support for individuals.
  • Providing advice, guidance, and support to service users, their families, and carers, empowering them to make informed decisions.
  • Maintaining accurate case records and documentation in accordance with statutory requirements and organizational policies.

To be considered for this position, you must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Current registration with Social Work England.
  • Demonstrable experience in adult social work, particularly in assessment and safeguarding.
  • Strong communication and interpersonal skills, with the ability to engage effectively with diverse populations.
  • Knowledge of relevant legislation and best practices in adult social care.
