The Role
As part of the Aging Well Adults' Community Team, you will support adults across the borough to maintain independence, wellbeing, and strong community connections through a strengths-based approach.
You will undertake assessments, develop personalised care plans, and work collaboratively with health professionals, community partners, and families to ensure individuals receive the right support at the right time.
Key Responsibilities
- Carry out strengths-based assessments and develop person-centred care and support plans
- Support individuals to maximise independence using community assets and networks
- Manage complex caseloads including safeguarding enquiries and Mental Capacity Act assessments
- Work collaboratively with health, police, and other partners to deliver coordinated support
- Promote innovative and creative social work practice within the team
- Maintain accurate records, reports, and communication with stakeholders
- Contribute to community engagement initiatives to reduce isolation and improve wellbeing
- Participate in quality assurance, audits, and ongoing professional development
About You
- Qualified Social Worker with Social Work England registration
- Experience in Adult Social Care, including Care Act 2014 and safeguarding
- Strong understanding of strengths-based and asset-based practice
- Experience working with complex cases and multi-agency partners
- Excellent communication, organisational, and decision-making skills
- Committed to continuous professional development and learning
