Are you a passionate and dedicated Social Worker looking to make a meaningful impact in the lives of adults within the community? North Lincolnshire is seeking a qualified Social Worker to join our dynamic Adults Community team. This role offers the opportunity to work collaboratively with individuals, families, and various stakeholders to promote independence and enhance the quality of life for our service users.
As a Social Worker in the Adults Community team, your key responsibilities will include:
Conducting assessments to identify needs and develop tailored care plans. Providing support and guidance to individuals and families, ensuring the best outcomes. Working closely with other professionals, including health services and community organizations. Advocating for the rights and needs of vulnerable adults. Monitoring and reviewing care plans, adapting them as necessary to meet changing needs.The ideal candidate will possess essential qualifications and skills, including:
A degree in Social Work or equivalent qualification. Registration with Social Work England. Experience in adult social care and a solid understanding of relevant legislation. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
