We are seeking a dedicated and compassionate Qualified Social Worker to join our Assessment & Intake Team in Southwark. This role is pivotal in ensuring that adults in need of support receive the appropriate assessments and interventions. You will work collaboratively within a multidisciplinary team to assess the needs of clients, formulate care plans, and ensure the delivery of high-quality services.
Your key responsibilities will include:
Conducting comprehensive assessments of individuals' needs and risks. Developing and implementing person-centered care plans in collaboration with clients and their families. Providing support and guidance to clients, ensuring their voices are heard in the decision-making process. Maintaining accurate and up-to-date case records in compliance with statutory requirements. Working closely with external agencies and stakeholders to coordinate services and resources.The ideal candidate will possess the following qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Proven experience in adult social work, particularly in assessment and intake processes. Strong communication and interpersonal skills, with the ability to build rapport with diverse clients.
