As part of your role, you will be responsible for:

  • Conducting comprehensive assessments of adults' needs and risks.
  • Developing and reviewing care plans in partnership with service users and their families.
  • Providing information and advice to service users regarding available support services.
  • Monitoring and evaluating the effectiveness of support services and interventions.
  • Maintaining accurate and up-to-date records in line with organizational policies.

To be considered for this position, you must possess the following qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team and manage a varied caseload.
