Join our dedicated Assessment Team in Harrow as an Adult's Social Worker, where you will play a vital role in supporting vulnerable adults in our community. We are seeking a passionate and qualified individual to conduct assessments, develop care plans, and provide ongoing support to ensure the well-being of our service users. You will work collaboratively with other professionals, families, and community organizations to promote independence and enhance the quality of life for those in need.
As part of your role, you will be responsible for:
- Conducting comprehensive assessments of adults' needs and risks.
- Developing and reviewing care plans in partnership with service users and their families.
- Providing information and advice to service users regarding available support services.
- Monitoring and evaluating the effectiveness of support services and interventions.
- Maintaining accurate and up-to-date records in line with organizational policies.
To be considered for this position, you must possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in assessment and care planning.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team and manage a varied caseload.
