We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Barnet. This role is vital in ensuring that adults receive the support and care they need during their hospital stay and upon discharge. You will work collaboratively with healthcare professionals, patients, and their families to assess needs, develop care plans, and facilitate access to services.

Key responsibilities include:

Conducting comprehensive assessments of individuals' social care needs in a hospital setting. Developing and implementing care plans that promote independence and well-being. Collaborating with multidisciplinary teams to ensure seamless transitions from hospital to community services. Providing support and guidance to patients and their families, addressing concerns and facilitating access to resources. Maintaining accurate and timely records in compliance with organizational policies and legal requirements.

Essential qualifications and skills for this position include:

A recognized social work qualification (e.g., BA/MA in Social Work). Registered with Social Work England. Proven experience in adult social work, preferably within a hospital environment. Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals. Excellent organizational and time management skills, with the ability to manage a varied caseload.
