The London Borough of Merton is seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work - Community/LD Team. This role is crucial in supporting individuals with learning disabilities and other complex needs, ensuring they receive the best possible care and support to lead fulfilling lives. As a member of our dynamic team, you will play a key role in promoting independence and empowering clients within the community.

Key responsibilities include:

Assessing the needs of adults with learning disabilities and developing tailored support plans. Working collaboratively with clients, families, and multi-disciplinary teams to ensure effective service delivery. Monitoring and reviewing care plans to adapt to changing needs and circumstances. Providing guidance and support to families, advocating for clients' rights and choices. Maintaining accurate and up-to-date case records in compliance with relevant legislation.

To be considered for this position, you must possess the following qualifications and skills:

A recognized social work qualification (e.g., BA/MA in Social Work). Registration with Social Work England. Experience working within adult social care, particularly with individuals with learning disabilities. Strong assessment and analytical skills, with the ability to manage complex cases. Excellent communication and interpersonal skills, with a commitment to partnership working.

Additional requirements for this role include a valid driving license and access to a vehicle, as travel within the borough may be necessary.
