Trafford - Adults Social Worker - Community Team

- Developing, implementing, and reviewing care plans in partnership with service users and their families.
- Providing support and guidance to individuals and their families, ensuring they understand available services.
- Collaborating with multi-disciplinary teams and external agencies to coordinate care and support.
- Maintaining accurate and timely records in compliance with local policies and procedures.
- Participating in safeguarding processes to protect vulnerable adults from harm.
To be considered for this position, you must possess the following qualifications and skills:
- A relevant Social Work qualification (BA/MA in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working with adults in a social work capacity, ideally within a community setting.
- Strong assessment and analytical skills, with the ability to manage complex cases.
- Excellent communication and interpersonal skills, with a commitment to person-centered practice.
- Knowledge of relevant legislation and best practices in social work.
