We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Community Team in Shropshire. In this vital role, you will work collaboratively with individuals and their families to promote well-being, safeguard vulnerable adults, and enhance their quality of life. You will be responsible for conducting assessments, developing care plans, and ensuring that services are tailored to meet the unique needs of each individual.

Your key responsibilities will include:

  • Conducting comprehensive assessments of clients’ needs and circumstances.
  • Developing, implementing, and reviewing care plans in collaboration with clients and their families.
  • Providing ongoing support and guidance to clients, helping them to access appropriate resources and services.
  • Working closely with multidisciplinary teams, including healthcare professionals and community organizations, to ensure holistic care.
  • Maintaining accurate and up-to-date records in accordance with legal and organizational standards.

To be considered for this role, you must possess the following qualifications and skills:

  • A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Experience in adult social work, particularly in community settings.
  • Strong assessment and analytical skills, with the ability to develop effective care plans.
  • Excellent communication and interpersonal skills, with a commitment to client-centered practice.
