In this role, your key responsibilities will include:

Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities and autism. Developing and implementing person-centered care plans in collaboration with service users, families, and multidisciplinary teams. Providing ongoing support and advocacy to ensure individuals access appropriate services and resources. Monitoring and reviewing progress, making necessary adjustments to care plans as required. Maintaining accurate and timely documentation in accordance with organizational policies and legal requirements.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with adults with learning disabilities and autism. Strong communication and interpersonal skills, with the ability to build trusting relationships. Knowledge of relevant legislation and best practices in social work.
