Join our dynamic Adult's SW - LD & Autism Team in Leicestershire, where you will have the opportunity to support individuals in achieving their goals and enhancing their quality of life. We are seeking a qualified social worker who is committed to promoting independence and well-being among vulnerable adults.
In this role, your key responsibilities will include:
Conducting comprehensive assessments to identify the needs and strengths of individuals with learning disabilities and autism. Developing and implementing person-centered care plans in collaboration with service users, families, and multidisciplinary teams. Providing ongoing support and advocacy to ensure individuals access appropriate services and resources. Monitoring and reviewing progress, making necessary adjustments to care plans as required. Maintaining accurate and timely documentation in accordance with organizational policies and legal requirements.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registered with Social Work England. Experience working with adults with learning disabilities and autism. Strong communication and interpersonal skills, with the ability to build trusting relationships.
