Your key responsibilities will include:
Conducting comprehensive assessments of clients' needs and circumstances. Developing and implementing personalized care plans in collaboration with clients and other professionals. Providing ongoing support and guidance to clients and their families. Working closely with multidisciplinary teams to ensure integrated service delivery. Maintaining accurate documentation and case records in compliance with legal and organizational standards.The ideal candidate will possess the following essential qualifications and skills:
A recognized social work qualification (e.g., BA/MA in Social Work). Current Social Work England registration. Proven experience in adult social work, particularly within community settings. Strong assessment and analytical skills.
