Key responsibilities include:
Conducting comprehensive assessments of individuals’ needs and developing tailored support plans. Collaborating with multidisciplinary teams to coordinate service delivery and ensure holistic support. Providing advocacy and guidance to individuals and their families, helping them understand available resources. Monitoring and reviewing progress to adapt support plans as necessary, ensuring positive outcomes. Maintaining accurate and up-to-date records in compliance with organizational policies and statutory requirements.The ideal candidate will possess the following qualifications and skills:
A relevant Social Work qualification (e.g., BA/MA in Social Work). Current registration with Social Work England.
