South Gloucestershire Council is seeking a dedicated and experienced Qualified Social Worker to join our Adult's Assessment Team. As a key member of our team, you will be responsible for conducting assessments, developing care plans, and ensuring the delivery of high-quality support services.

Your key responsibilities will include:

Conducting comprehensive assessments of individuals' needs and risks, ensuring that all relevant information is gathered and analyzed. Developing and implementing person-centered care plans in collaboration with clients, families, and other professionals. Providing ongoing support and review of care plans to ensure they meet the evolving needs of clients. Maintaining accurate and timely documentation in accordance with statutory requirements and council policies. Collaborating with multidisciplinary teams and external agencies to facilitate effective service delivery.

The ideal candidate will possess the following essential qualifications and skills:

A recognized Social Work qualification (BA/MA in Social Work or equivalent). Registration with Social Work England. Proven experience in adult social work, particularly within assessment and care planning. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Excellent organizational skills and the ability to manage a caseload effectively.
