Your key responsibilities will include:

Conducting comprehensive assessments of individuals’ needs and circumstances. Developing and implementing care plans in collaboration with service users and their families. Providing ongoing support and advocacy to clients, ensuring their voices are heard. Working with multidisciplinary teams to coordinate services and promote positive outcomes. Maintaining accurate records and documentation in compliance with legal and organizational standards.

The ideal candidate will possess the following essential qualifications and skills:

A recognized social work qualification (degree in social work or equivalent). Current registration with Social Work England. Strong understanding of relevant legislation and best practices in adult social care. Excellent communication, interpersonal, and organizational skills. Ability to work independently and as part of a team, managing a varied caseload.
