Southampton - Adults Social Worker - Connect Team

- Monitoring and reviewing care plans to ensure they meet the changing needs of service users.
- Working closely with multidisciplinary teams and external agencies to coordinate services and support.
To be successful in this role, you will need the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly within a community setting.
- Strong assessment and analytical skills, with the ability to develop tailored care plans.
- Excellent communication and interpersonal skills, with a compassionate approach to service delivery.
