We are seeking a dedicated and compassionate Social Worker to join our Acute Hospital team in Shropshire. As a Social Worker in our acute hospital setting, you will provide essential support, advocacy, and guidance to patients facing complex health and social challenges.
Your key responsibilities will include:
Conducting comprehensive assessments of patients' social, emotional, and environmental needs. Collaborating with multidisciplinary teams to develop and implement effective care plans. Providing crisis intervention and emotional support to patients and their families. Facilitating discharge planning and connecting patients with appropriate community resources. Maintaining accurate and timely documentation of casework and interventions.The ideal candidate will possess the following essential qualifications and skills:
A recognized degree in Social Work (BA or MA) and current registration with Social Work England. Proven experience in a healthcare or acute hospital environment. Strong interpersonal and communication skills, with the ability to build rapport with diverse populations. Excellent problem-solving skills and the capacity to work effectively under pressure.
