Adult's SW - Hospital Discharge Team - Darlington

We are seeking a dedicated Adult's Social Worker to join our Hospital Discharge Team in Darlington. As a key member of the team, you will be responsible for facilitating safe and timely discharges for adult patients, ensuring they receive appropriate support and services to transition back to their communities.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs of patients prior to discharge
  • Developing and implementing comprehensive discharge plans in collaboration with multidisciplinary teams
  • Arranging and coordinating post-discharge services, such as home care, rehabilitation, and support groups
  • Advocating for patients to access necessary resources and services to promote their well-being
  • Providing emotional support and guidance to patients and their families during the discharge process
  • The ideal candidate will possess:

    • A recognized Social Work qualification and registration with Social Work England
    • Demonstrated experience in adult social work, particularly in hospital discharge planning
    • Strong assessment and case management skills, with the ability to work effectively under pressure
    • Excellent communication and interpersonal skills to engage with patients, families, and external agencies
    • Additional requirements:
