We are seeking a dedicated and compassionate Community Adult Social Worker to join our team in Wolverhampton. As a key member of our Qualified Social Work department, you will be responsible for providing support and assistance to vulnerable adults in the community.

Key responsibilities and duties include:

Conducting assessments to determine the needs of adult service users Developing and implementing care plans to support individuals in achieving their goals Providing advocacy and support to help service users access necessary resources and services Working closely with other healthcare professionals and agencies to ensure holistic care for service users Monitoring and reviewing the effectiveness of care plans and making adjustments as needed

The ideal candidate will possess the following essential qualifications and skills:

Qualified Social Worker with relevant experience in adult social care Registration with Social Work England Strong communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team Empathy, patience, and a non-judgmental approach to working with vulnerable adults

Additional requirements for this role include a valid driver's license and access to a vehicle, as the position may involve home visits and travel within the community.
