We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Hospital & Community Team in Portsmouth. This role offers a unique opportunity to make a positive impact on the lives of vulnerable individuals in both hospital and community settings.

Key responsibilities and duties:

  • Assessing the needs of patients in hospital and developing care plans
  • Providing support and guidance to patients and their families to ensure a smooth transition back to the community

  • Advocating for the rights and best interests of patients, particularly those who are marginalized or at risk

Essential qualifications and skills:

  • Qualified Social Worker with a valid registration with Social Work England
  • Experience working in a hospital or community setting, ideally with a focus on adult social care

  • Ability to work autonomously and as part of a multidisciplinary team
