We are currently seeking a dedicated and experienced Qualified Social Worker (QSW) to join our Hospital & Community Team in Portsmouth. This role offers a unique opportunity to make a positive impact on the lives of vulnerable individuals in both hospital and community settings.

Key responsibilities and duties:

Assessing the needs of patients in hospital and developing care plans Providing support and guidance to patients and their families to ensure a smooth transition back to the community Collaborating with healthcare professionals, social services, and other agencies to coordinate comprehensive care Advocating for the rights and best interests of patients, particularly those who are marginalized or at risk Maintaining accurate and up-to-date records of interventions and progress

Essential qualifications and skills:

Qualified Social Worker with a valid registration with Social Work England Experience working in a hospital or community setting, ideally with a focus on adult social care Strong assessment and communication skills to engage effectively with diverse populations Ability to work autonomously and as part of a multidisciplinary team Commitment to upholding professional standards and ethical practice
