We are currently seeking a dedicated and experienced Adult's Social Worker to join our Hospital Team in North Lincolnshire. As a key member of our team, you will be responsible for providing high-quality social work services to adults in a hospital setting.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs of adult service users
  • Developing and implementing care plans in collaboration with multidisciplinary teams
  • Providing support and guidance to individuals and their families
  • Advocating for the rights and best interests of service users
  • Participating in safeguarding procedures and risk assessments

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration
  • Demonstrated experience in adult social work, preferably within a hospital or healthcare setting
  • Strong assessment and care planning skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively within a multidisciplinary team
