We are currently seeking a dedicated and experienced Social Worker to join our team at Westminster Adults Hospital. As a key member of the Qualified Social Work field, you will play a crucial role in providing support and guidance to patients and their families during their time at the hospital.
Key responsibilities and duties:
- Conducting assessments to determine patients' needs and develop care plans
- Providing counseling and emotional support to patients and their families
- Liaising with healthcare professionals to ensure holistic care for patients
- Advocating for patients' rights and ensuring their voices are heard
- Assisting with discharge planning and coordinating post-hospital care services
Essential qualifications and skills:
- Qualified Social Worker with relevant experience in a healthcare setting
- Registration with the Health and Care Professions Council (HCPC)
- Strong communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team
- Empathy, compassion, and a non-judgmental approach to patient care
