We are currently seeking a dedicated Adult's Social Worker to join our Hospital Team in Portsmouth. As a key member of the team, you will be responsible for providing high-quality social work services to adults in a hospital setting, ensuring their well-being and promoting independence.

Key responsibilities and duties include:

  • Conducting assessments to determine the needs of adult patients
  • Developing care plans and interventions to support individuals in achieving their goals

The ideal candidate will possess the following essential qualifications and skills:

  • Qualified Social Worker with a relevant degree and HCPC registration
  • Demonstrated experience in adult social work, preferably in a hospital or healthcare setting
  • Strong assessment and case management skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively in a fast-paced and challenging environment

Additional requirements for this role include a valid driver's license and access to a vehicle for travel between hospital and community settings.
