We are seeking a passionate and dedicated Qualified Social Worker to join our Adult's Social Work - Learning Disabilities Team in Worcestershire. You will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and ensure the delivery of high-quality support services.

Key responsibilities include:

  • Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and preferences.
  • Developing, implementing, and reviewing person-centered care plans in collaboration with clients and their families.
  • Providing ongoing support and advocacy to clients, ensuring their rights and choices are respected.
  • Working closely with multidisciplinary teams, including healthcare professionals, to deliver integrated care.
  • Maintaining accurate and up-to-date records in compliance with legal and organizational standards.

To be successful in this role, candidates must possess the following qualifications and skills:

  • A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Strong understanding of legislation and best practices related to adult social care and learning disabilities.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
  • Proven experience in adult social work, particularly in supporting individuals with learning disabilities.
