We are seeking a passionate and dedicated Qualified Social Worker to join our Adult's Social Work - Learning Disabilities Team in Worcestershire. This role offers an exciting opportunity to make a positive impact on the lives of adults with learning disabilities, supporting them to lead fulfilling and independent lives. You will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and ensure the delivery of high-quality support services.
Key responsibilities include:
Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and preferences. Developing, implementing, and reviewing person-centered care plans in collaboration with clients and their families. Providing ongoing support and advocacy to clients, ensuring their rights and choices are respected. Working closely with multidisciplinary teams, including healthcare professionals, to deliver integrated care. Maintaining accurate and up-to-date records in compliance with legal and organizational standards.To be successful in this role, candidates must possess the following qualifications and skills:
A relevant Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Current registration with Social Work England. Strong understanding of legislation and best practices related to adult social care and learning disabilities. Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
