We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work - Integrated Multi-Agency Care (IMC) Team in Bolton. This role is pivotal in ensuring the delivery of high-quality social care services to adults in our community. The successful candidate will work collaboratively with a range of professionals to provide holistic support to individuals and their families, promoting independence and well-being.

Key responsibilities include:

Conducting comprehensive assessments of individuals' needs and risks, developing and implementing tailored care plans. Collaborating with other agencies, including health services, to ensure a coordinated approach to care. Providing advice, support, and advocacy to service users and their families, empowering them to make informed decisions. Maintaining accurate and up-to-date case records, ensuring compliance with relevant legislation and policies. Participating in regular team meetings and contributing to the continuous improvement of service delivery.

To be considered for this role, candidates must possess the following essential qualifications and skills:

A recognized Social Work qualification (Degree in Social Work or equivalent). Registered with Social Work England. Demonstrable experience in adult social work, particularly within an IMC setting. Strong assessment, communication, and interpersonal skills. Ability to work effectively under pressure and manage a diverse caseload.
