Bracknell - Social Worker - Learning Disability and Autism Team
We are seeking a dedicated and compassionate Social Worker to join our Learning Disability and Autism Team in Bracknell. Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs of individuals with learning disabilities and autism.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Providing ongoing support and advocacy to individuals, ensuring they have access to appropriate services and resources.
- Working closely with multidisciplinary teams to coordinate care and support effectively.
- Maintaining accurate and up-to-date case records in accordance with legal and organizational requirements.
The ideal candidate will possess the following qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, CQSW, CSS, or equivalent).
- Registered with Social Work England.
- Experience working with individuals with learning disabilities and autism.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with service users and families.
- Excellent problem-solving skills and the ability to work independently as well as part of a team.
