Are you a passionate and dedicated Qualified Social Worker looking to make a difference in the lives of individuals and families? We are seeking a skilled QSW to join our Hospital Team in Bracknell. In this vital role, you will work collaboratively with healthcare professionals to provide comprehensive social care assessments, support discharge planning, and ensure the best outcomes for patients transitioning from hospital to home or alternative care settings.

Your key responsibilities will include:

Conducting thorough assessments of patients' social care needs and risks. Developing and implementing care plans in collaboration with multidisciplinary teams.
