We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Safeguarding/Assessment Team in Bracknell. This is an exciting opportunity for an individual who is committed to promoting the well-being and safeguarding of vulnerable adults. You will play a vital role in assessing needs, developing care plans, and ensuring the safety and welfare of service users in our community.
Key responsibilities include:
Conducting comprehensive assessments of adults' needs and risks, ensuring adherence to safeguarding policies and procedures. Developing and implementing person-centered care plans in collaboration with service users, families, and other professionals. Monitoring and reviewing care plans to ensure effective service delivery and positive outcomes. Providing support and guidance to service users, empowering them to make informed choices about their care. Maintaining accurate and up-to-date records in compliance with local and national standards. Collaborating with multidisciplinary teams to ensure a holistic approach to service delivery.To be considered for this role, candidates must possess the following essential qualifications and skills:
A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent). Registration with Social Work England. Proven experience in adult safeguarding and assessment within a social work setting. Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders. Excellent organizational skills and the ability to manage a caseload effectively.
