We are seeking a dedicated and compassionate Hospital Social Worker to join our dynamic team in Lambeth. In this vital role, you will work collaboratively with healthcare professionals to provide essential support to patients and their families during critical times. You will be responsible for assessing patient needs, developing care plans, and facilitating access to resources that promote health and well-being.

Your key responsibilities will include:

Conducting psychosocial assessments and evaluations to determine the needs of patients and their families. Developing and implementing individualized care plans in collaboration with medical staff and other professionals. Providing emotional support and counseling to patients and their families, helping them navigate complex healthcare systems. Coordinating discharge planning and follow-up services to ensure continuity of care. Advocating for patients' rights and access to necessary services and resources.

The ideal candidate will possess the following essential qualifications and skills:

A recognized qualification in Social Work (e.g., BA/MA in Social Work). Current registration with the relevant professional body (e.g., Social Work England). Proven experience in a hospital or healthcare setting, preferably with a focus on social work. Strong interpersonal and communication skills, with the ability to work effectively in a multidisciplinary team. Empathy, resilience, and a commitment to providing high-quality care to vulnerable populations.
