We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - Learning Disabilities & Autism Team in Bracknell. This role is essential in providing support and advocacy for adults with learning disabilities and autism, ensuring they receive the care and services they need to lead fulfilling lives. As a key member of our team, you will work collaboratively with individuals, families, and other professionals to develop tailored support plans and promote independence.

Your key responsibilities will include:

  • Conducting assessments and developing care plans for adults with learning disabilities and autism.
  • Providing ongoing support and intervention, ensuring the best outcomes for service users.
  • Working closely with families, carers, and external agencies to coordinate services and resources.
  • Monitoring and reviewing care plans to ensure they remain effective and relevant.
  • Maintaining accurate records and documentation in accordance with local policies and procedures.

To be considered for this role, you must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, or DipSW).
  • Registered with Social Work England.
  • Experience working with adults with learning disabilities and autism.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
  • Excellent organizational skills and the ability to manage a varied caseload effectively.
