Senior Practitioner - Young Adults Transitions - Bristol

As a Senior Practitioner, your key responsibilities will include:

  • Developing and implementing personalized transition plans for young adults, focusing on their unique needs and aspirations.

  • Conducting assessments and evaluations to identify risks and opportunities for young adults in transition.
  • Building strong relationships with young adults, their families, and external agencies to facilitate effective support systems.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Significant experience working with young adults, particularly in transition settings.
  • Strong knowledge of relevant legislation, policies, and best practices in social work.
  • Excellent communication and interpersonal skills, with an ability to engage and motivate young people.
